Registration Form for the O’Connell Family Fundraiser
Come out to support and encourage the O’Connell Family in their fight against Gina’s breast cancer. This fundraiser gives us all the chance to rally around Chris and Gina while we raise money for them during their time of need. Lady donators will participate in one of Gina’s favorite hobbies - creating hand stamped cards for loved ones. Gentlemen donators will be entered into a 50/50 raffle and will also have the chance to spend time with Chris while enjoying games of ping pong and foosball.

Time: 10am-1pm
**Ladies should plan to arrive by 11am in
order to complete all projects before 1:00.

**Most of the parking is on the street either in front of the building or behind it.
Registration Fee: $40 (with ½ of every registration going directly to The O’Connell Family)

Ladies Registration Upon arrival, ladies will receive a packet which includes adhesive and all the other pre-cut materials and supplies needed to create 6 all-occasions greeting cards for yourself and a 7th card for Gina. |
Gentlemen’s Registration Upon arrival, gentlemen will receive a ticket to be entered into a 50/50 raffle. Gentlemen can then spend their time socializing with Chris and enjoying games at the church, such as ping pong and foosball. |

Things To Know:
· It’s okay if you don’t consider yourself to be crafty or have never stamped before! We will have experts to assist you at each table. We promise it’s a fun activity that you’ll enjoy trying out for the first time!
· Food and drinks will be provided.
· There will also be raffle baskets and other items for more donation opportunities.
· A Love and Encouragement Signature Quilt is being made for Gina. As part of your registration fee, every donator in attendance will receive a quilt block to sign and personalize with a message for Gina.
· Attendees are encouraged to wear pink in support of Breast Cancer Awareness.
· If you live too far away or are unable to attend, you can still participate! Turn in the registration form below with your $40 and we will enter your name into the Gentlemen’s 50/50 Raffle.
